HEALTH chiefs are trying to make life easier for people when they are discharged from hospital.
The Harrogate and District NHS Foundation Trust has joined forces with social services to provide information in a more user friendly and accessible manner.
Tomorrow, a multi-media information display will be held in the foyer of Harrogate District Hospital, and staff will be available to offer advice and information.
The aim is to ensure patients and their carers are aware of the range of options available following discharge from hospital in order for them to make an informed choice about choosing the most appropriate care for their needs. By working together, the trust and social services hope to improve patient care and make the process much less stressful.
The display will also remain in the main entrance foyer this weekend.
